1. Introduction to the four steps of rehabilitation care provision


The provision of rehabilitation interventions is done in a stepwise manner.

There are four major steps involved in providing interventions for rehabilitation.

The steps enable the person to:

  • Receive the most appropriate rehabilitation intervention that meets their rehabilitation needs
  • Be informed about the rehabilitation intervention and if possible, how to practice self-management at home
  • Receive rehabilitation support in terms of monitoring, follow-up and referral to other specialists

The four steps of rehabilitation intervention provision are:

  1. Assess for the difficulty area
  2. Plan the appropriate care for rehabilitation
  3. Deliver the care for rehabilitation
  4. Monitor the care provided
